We hope your first visit to our practice will lead to a relationship of many years where you develop a strong bond with your GP and the nursing and receptionist staff who care for you and your family.

Our practice serves the local community, keeping you healthy by appropriate medical treatment when needed and sharing advice on how to maintain good health and prevent illness whenever possible.

As a teaching practice we often host registrars and students, they’re fully supported by our experienced staff and always full of enthusiasm and eagerness to excel at general practice.

Not every type of GP suits every patient. Lifelong Healthcare offers you a team of GPs with a wide range of both medical interests and experience. This means you’ll find a GP who you’ll be able to build rapport with, trust, talk to and develop what we hope is a strong long-term relationship. It doesn’t matter what life stage you are at, whether you have ongoing medical issues or just need a quick consultation.

Dr Nigel Berry

M.B.B.S.


Dr Berry has been in general Practice at this surgery since 1976. His experiences as a Family Practitioner include all areas of family medical care. Dr Berry has a special interest in Aged Care visiting a number of Residential Aged Care Facilities weekly.

Dr Robert Smith

M.B. Ch.B


Dr Smith achieved his medical degree in the UK and has been in General Practice at this surgery since 1988. As a Family Practitioner he sees many aspects of family care including mens and women’s health, mental health and children’s health.

Dr Carolyn Royse

M.B.B.S F.R.A.C.G.P Honorary Clinical Lecturer, Dept of General Practice, Univerity of Melbourne


Dr Royse has been in general practice for the last 18 years. She is available for all types of general practice including shared antenatal care, women and child health, mental health, preventative health and aged care. Dr Royse has a particular interest in cosmetic procedures including botox, juvederm, laser procedures for skin conditions and in medical education. Dr Royse is a clinical lecturer at the University of Melbourne, Department of Medicine.

Dr Roheela D’Cruz

M.B.B.S BSc (Med), FRACGP, MPH, DCH, Dip Pall Med


Dr Roheela D’Cruz graduated from the University of NSW and started at this practice in January 2013. As well as seeing all areas of family medicine, Dr D’Cruz has a special interest in paediatrics, preventative health and palliative medicine.

Dr Jacqueline Laing

M.B.B.S F.A.M.A.C


Dr Laing has been a General Practitioner at this surgery since 1982 and sees all areas of family health including women and children’s health, mental health and aged care. She has a special interest in acupuncture and cosmetic procedures including botox, juverderm and laser procedures for skin conditions.

Dr Alicia Polakiewicz

M.B.B.S Dip Nutrn & Enviro Med


Dr Polakiewicz has been in General Practice at this surgery since 2003. Prior to this she has worked in the Royal Flying Doctor Service, North West W.A. and in remote Aboriginal Communities in N.T. and has been in General Practice since 1983. Dr Polakiewicz has experience in all areas of family medicine with a special interest in nutritional medicine.

Dr Andrew Mason

M.B.B.S Dip. Obs RACOG


Dr Mason has been in General Practice at this surgery since 1987. As a Family Practitioner he sees many aspects of family medicine. He does have a special interest in travel medicine and has completed ISTM (International Society of Travel Medicine) Certificate of Travel Health.

Dr Tony White

M.B.S F.R.A.C.G.P


Dr White graduated from University of Melbourne in 1993 and has been practising as a General Practitioner at this practice since 2007. As well as seeing all areas of family medicine Dr White has a special interest in travel medicine.

Dr Mary Ang

M.B.B.S


Dr Ang graduated from the University of Melbourne and joined this practice in 2014. She has worked in various hospitals in Melbourne including Box Hill hospital, Dandenong Hospital and Monash Health.

She enjoys all aspects of general practice such as diabetes, asthma and preventative health.

Dr Adam Wade

M.B.B.S


Dr Wade graduated from UWA Perth in 2001 and has extensive hospital and critical care experience prior to joining the practice in 2014.
He enjoys all aspects of General Practice with a special interest in paediatrics, men’s health, chronic disease and anti-wrinkle treatments.
When not at work he occasionally finds time to escape to the hills on a bicycle.

Dr Maheshy Sooriyakumaran

M.B.B.S


Dr Maheshy graduated from the University of Melbourne in 2011 and completed further training at Eastern Health before starting in general practice. She enjoys all aspects of family medicine, and has a particular interest in women’s and children’s health, and mental health.

Dr Asha Ford

M.B.B.S


Dr Asha Ford completed her medical degree at Monash University, having previously graduated from a Bachelor of Applied Science (Biomedical Science) and Honours at RMIT University. Following medical school, Dr Ford worked at Frankston and Rosebud Hospitals gaining experience across various specialties and is now undertaking GP fellowship training with The Royal Australian College of General Practitioners. She enjoys all aspects of general practice.
